Request for attendance forms - Instructions

The form is divided into 4 parts:

  • General information about the Conference/Workshop:  Title, Date, Location, etc. (located at the top of the form.)   Principals - Be sure to indicate account # and title.

 

  • Costs and Estimated Reimbursable Expenses (Middle of form, left hand column) - All expenses must be listed - teacher sub costs are 77.00/day.

  • Registration Process (Middle of form, right hand column) - a registration form must be attached, and a method of registration must be indicated.  Principal must sign at bottom of this column indicating approval of registration expense and employee being absent from building to attend.  

  • Reimbursement of Actual Expenses (Box at bottom of form) - after attending conference, this box should be completed on the pink copy of the form, itemizing any expenses to be reimbursed.  Original itemized receipts must be attached, and principal must sign in the box to approve the reimbursement of these expenses.  (Registration cost should only be listed at bottom if employee paid the fee themselves.) Meal receipts should list food items purchased, and must not contain any alcohol.  Meal cost limits are listed on the conference form.

DESIGN:    Staff development that improves the learning of all students uses learning strategies appropriate to the intended goal.


LEARNING:    Staff development that improves the learning of all students applies knowledge about human learning and change.


COLLABORATION:  Staff development that improves the learning of all students provides educators with the knowledge and skills to collaborate.


Content Standards

EQUITY:    Staff development that improves the learning of all students prepares educators to understand and appreciate all students, create safe, orderly and supportive learning environments, and hold high expectations for their academic achievement.


QUALITY TEACHING:  Staff development that improves the learning of all students deepens educators' content knowledge, provides them with research-based instructional strategies to assist students in meeting rigorous academic standards, and prepares them to use various types of classroom assessments appropriately.


FAMILY INVOLVEMENT:    Staff development that improves the learning of all students provides educators with knowledge and skills to involve families and other stakeholders appropriately.
